Instructions for Consignees

• Download this form and print it out.
• Consignee ID number will be assigned when your merchandise is checked in.
• Remember to print carefully so your check will be mailed to YOUR house.
• In the upper section, fill out your last name, first name, address, city, zip code, cell phone number (important), then date and sign the form.
• In the lower section, list each item for sale on its own line.
• Complete the description and price, and check the box if you want to donate the item if it doesn’t sell.
• A bar code number will be assigned at check-in for each item, so leave it blank for now.
• Bring your completed and signed form along with your items to check-in during the times listed below.
